The DbDataReader.GetTextReader method should fail like DbDataReader.GetStream when the result is returned as an xml. Instead, each TextReader returns a part (a network packet?) of the result.

Inconsistency when indenting code - by Bruno Martinez
int h = 0; static_cast<int>(0) - 0; static_cast<int>(h)-h; is indented with spaces around "-" for the second line but not the third.

interior_ptr should be iterators - by Bruno Martinez
If interior_ptr was an iterator, we could convert a clr string to a wstring easily and without needing to pin the clr string. I failed specializing iterator_traits<cli::interior_ptr<_Ty>>myself.

According to 23.4.43 map element access, std::map's operator[] requires mapped type to be DefaultInsertable. Visual C++ requires it to also be copyable.
