Description

This was previously reported in earlier SQL releases (see item #416197) but was supposed to be fixed in 2012.

However, I have a database running on SQL 2012 SP2 Standard Edition running on Windows Server 2012 R2 Standard Edition in a VMware virtualised environment (NOTE: it is SP2 and Windows Server 2012 R2 but that service pack and OS are not included in the version/OS drop-downs on the page below) in which the DMVs still report an index is missing even though it precisely matches an existing (and trusted) index. 

One slightly unusual feature of the index is that it has no equality columns - just a single inequality column and a bunch of included columns.
