This bug is to do with the Visual Studio API, It's causing issues with my solution explorer tools extension -
The lazy loading of collapsed nested solutions causes queries to the API to throw exceptions and return incorrect data. As soon as you expand the folder (manually, can't do it in code) everything gets fixed.

Attached is an example which  shows off the cases

Lazy loading is a performance measure. To work with them, you can expand the items first using IVsUiHierarchyWindow and IVsUiHierarchy, see for example:
